The figures below are for a single adult with no dependants and no semi-private hospital, and they match PolicyAdvisor's published table on all six overlapping cells, which incidentally reveals that unlabelled table is Ontario single-rate. Two things stand out. Adding dental to Enhanced in Ontario at 45-54 costs $109.86 a month, or $1,318 a year, to buy $750 preventive plus $500 restorative plus a $1,500 lifetime orthodontic maximum — you can lose money on that unless you use the ortho. And Ontario runs consistently above Alberta and BC, with Enhanced plus dental at 45-54 costing $282.53 in Ontario against $249.40 in Alberta and $238.98 in BC.
